Note: SCHEDULED OUTAGE--On Tuesday, May 24, 2005, Oregon DMV images will be unavailable, beginning at 0700 local time, for approximately ten minutes while a new firewall is brought up. Obviously, the potential for a longer service disruption exists any time work is being done on the system. However, in this case, impact upon users is expected to be minimal.

The information below is for law enforcement use only. Access is logged. To gain access to WINPHO, take the following FOUR steps:

1. UPDATED (7 March 2005)! To subscribe to WINPHO please fill out this online form. We no longer use the printed form.



2. Complete the User Agreement. The new WINPHO User Agreement is available here. This agreement is mandatory and is not amendable by the agency. Please print the agreement and have it signed by someone in your agency who has the authority to bind the department on criminal history information security issues. Keep a copy for your records, and send the original, signed document to:

3. (Optional) If you'd like access to Oregon DMV data, you must be make the proper arrangements with DMV itself. ODOT charges $208 per year for unlimited access via WINPHO. This charge is in addition to WINPHO fees payable to WIN. If your agency would like access to Oregon DMV images, please download this form, complete it, and submit it to DMV for approval. Note that, while electronic access to Oregon DMV images is provided exclusively by WIN through WINPHO, ODOT continues to service non-WINPHO customers in the traditional manner.



4. To register users, please save this Microsoft Excel spreadsheet to your local hard drive (right click on the link and select "Save Target As," then in the dialog box save the spreadsheet to a location on your computer), complete the information for each user, and email the spreadsheet to WIN at the address indicated.
